If you choose to go through insurance, please bring your insurance card and driver's license with you to your appointment. We will use these to verify your insurance coverage and gather the information necessary to secure your potential insurance coverage specific to our chiropractic clinic.

Your insurance coverage will then be reviewed and explained to you in GREAT detail during your follow-up, second appointment, along with how your insurance coverage influences your chiropractic care at RAK Chiropractic.

Until we have been able to complete your insurance verification process, you will be required to pay for your care upfront. Once all insurance information has been gathered and your insurance coverage has been verified, any payments you have made will be applied to your co-payment, co-insurance, deductible, or uncovered services. This could result in a portion or a credit. Coverage will then be confirmed upon the review of your Explanation of Benefits, once they are received. If you have 100% coverage, payments will be refunded.

There may be guidelines, limitations, and restrictions with insurance that you may be unaware of.
Most insurance companies use an additional insurance management company. These insurance management companies are there to manage, regulate, and monitor insurance claims. This means there may be additional restrictions and limitations with your current coverage.

Some services at RAK Chiropractic could be considered not medically necessary, according to many health plan policies. Many health care plans deem the use of spinal and extra-spinal manipulation/adjustments for non-musculoskeletal conditions to be unproven and therefore not covered under your insurance.

Most health policies are designed and intended to only take care of acute conditions deemed ‘medically necessary’. Insurance will only cover painful, neuromuscular skeletal conditions caused by subluxations preventing your daily living activities.

The doctors at RAK Chiropractic can only treat an area of subjective complaint (where you indicate pain), and you must follow a treatment plan supporting acute care.

Any new or different complaint(s) requires administration, consultation, examination FEES all followed by a NEW acute treatment plan.

For every modality, therapy, or service rendered and/or provided, there will be fees, and both you and your insurance will be billed accordingly. This means the patient fees typically vary in price.

Your appointment schedule CANNOT resemble any sort of ‘maintenance’ schedule.
This also includes continuing care for the original problem.

INFANT CARE
The use of spinal and extra-spinal manipulation/adjustments for non-musculoskeletal conditions is NOT COVERED.
